| D | Mode of transportation ( 1 letter, ex. J (truck), R (railroad)) |
| E | Originating state ( 2 letters, ex. NE ) |
| F | Destination state ( 2 letters, ex. NE ) |
| G | Terminal location ( ex. Sidney, if no terminal, leave blank)
Will be used as destination terminal location for schedules 2A and 4 |
| H | Terminal number ( 9 characters, ex. T47NE3604, or leave blank)
Will be used as destination terminal numberfor schedules 2A and 4 |
| I | Buyer or Seller name ( ex. Texas Oil Inc. ) |
| J | Buyer or Seller FEIN or SSN ( 9 digits, ex. 470101010) |
| K | Date ( format M/D/YYYY preferred, ex. 9/30/2009 ) |
| L | Document # ( up to 10 characters ) |
| M | Gallons of product ( numeric, ex. 1500 ) |
| N | Product code (include leading 0s, ex. 065 ) |
