your comma separated file (.csv) must contain columns A-T. Column U should be used if schedule could be used on various returns.
Columns V and W should be included for schedule 15C (inventory) only.
Arrange your spreadsheet in the following format. Do not include
column headers (descriptions) in your file. Unused fields can
be left blank. |

| B | Carrier number (9 digits, ex. 123456789) |
| C | Mode of transportation (1 letter, ex. J (truck), R (railroad)) |
| D | Originating state (2 letters, ex. AL) |
| E | Terminal# (9 characters, ex. T72AL2345, required for 15A, 15B, 15C) |
| F | Destination state (2 letters, ex. AL) |
| G | Buyer or Seller name (ex. Alabama Oil Inc.) |
| H | Buyer or Seller number (9 digits, ex. 123456789) |
| I | Date (format M/D/YYYY, ex. 8/31/2010) |
| J | Document/Manifest number (up to 10 characters) |
| K | Product code (include leading 0's, ex. 065) |
| L | Net gallons (numeric, ex. 1500) |
| M | Gross gallons (numeric, ex. 1500, or 0) |
| N | Schedule code (up to 3 characters, ex. 5A) |
| O | Consignor or Position Holder name (ex. Southern Oil) |
| P | Consignor or Position Holder number (9 digits, ex. 123456789) |
| Q | Process this load with a different month (normally 0) |
| R | DeliveredTo name (transporter report only) |
| S | DeliveredTo number (9 digits, transporter report only) |
| T | DeliveredTo address (no commas, transporter report only) |
| U | Supplier/Distributor Report Type (1st letter of report type - 'S','I','E') |
